FORMS AND DOCUMENTS TO BE PREPARED BEFORE THE ACTIVITY

• The acceptance confirmation received from host institution (Letter of Acceptance)
• The Grant Agreement and its Annexes signed between the student and the higher education institution ( The bottom-right corners of each page in the agreement must be signed and the red areas muct be filled.) ( Erasmus ID Codes)

 Annex-2 : It is not obligatory to have the General Conditions in a copy  to be kept at the university.       

Annex-3 : Erasmus Student Charter

3- Account Declaration Petition

4- Renunciation Form

5- Dormitory Petition  (For students leaving the dormitory) 

6- Copy of Passport  (Visa page , Identity information page) – Flight Details: Flight ticket , reservation document etc..  

7- Conformity Letter for Master’s/ Doctorate Students

8- A copy of the health insurance Policy (CLICK for more information)

9- Commitment

10- Approval Letter given by the department for students who wants to make an extension.

11- Special Needs Support – Additional Grant Request Form

12- Online Language Support: Students should complete the 1st online language assessment before starting the mobility

DOCUMENTS TO BE COMPLETED AFTER MOBILITY HAS ENDED

1. Transcript of Records prepared according to the ''European Credit Transfer System'' related to the stuy period at host institution
2. Certificate of Attendance
3. Photocopies of pages with the entry-exit seals
4. Online Learning Agreement - After the mobility. ( It should be filled and approved by the host institutions and given to the student to show the student's mobility starting and end date. Learning Agreement After the Mobility. If a separate document showing the activity dates is issued and approved by the counterparty, these documents are also accepted.

5. Student final report: By using the Beneficiary Module , students are asked to complete the online EU Survey.Students whose activities are completed are required to fill out the questionnaire.
