- The explanations required for each section in the application form must be filled in completely.
- Certificate of Use of Experimental Animals; It must have been taken as a result of the 80-hour 
course program within the framework of the Regulation on the Working, Procedures and Principles of
 Animal Experiments Ethics Committees of the Ministry of Environment and Forestry. The person whose 
certificate is presented in the application must take part in the project as a researcher.
- All researchers' signatures on the Ethics Committee Application Form and the project coordinator's 
signatures on the Commitment Note must be complete. Applications with missing signatures 
willnot
 be accepted.
- In universities and educational institutions, it is required to be an executive and a supervisor 
for theses.
- 1 (one) CD copy should be prepared in pdf format containing the application form and the
undertaking.
- 2 pieces of literature will be added to the back of the application form.
- Application form, certificate of use of experimental animals and literature will be combined and 
presented in a single pdf.
The application form should be copied to a CD in the word file format (in addition to the pdf file).

After these conditions are met, the application form can be submitted to the Selcuk University 
Experimental Medicine Application and Research Center Ethics Committee Secretariat by placing 
the 
application form, the undertaking, the CD and the original copy of the experimental animals use 
certificate, in a plastic-covered file.

IMPORTANT: SÜDAM Ethics Committee will examine the first 8 projects applied for each term.
